Case Management and Stabilization services

MEDICAL CASE MANAGEMENT

· Work with healthcare providers to manage clients' medications.
·Help coordinate and apply for SSDI, General Relief, and SNAP.
·Provide assistance, coordination, and facilitation for clients in applying for health insurance programs, including Medicaid and Medicare.
Help clients understand complex health care programs.
·Federally Qualified Health Center (FQHC) application
Provide healthcare guidance and client advice.

NON -MEDICAL CASE MANAGEMENT

· Assist and support clients with housing, rental assistance, and eviction referrals.
· Assist and support clients with skills training, employment, and counseling referral.
· Assist and support clients with information and resources on community food pantries and services.
Connect clients with transportation services to help them attend medical appointments, stay in care, and access medical services.
